Quality is the share of units produced that met specification first time. Rework counts against it. A unit that needed a second pass consumed capacity twice, even though it eventually shipped.

Quality rate questions

Does rework count as a defect?

Yes, in the OEE definition. The unit failed on the first pass and consumed capacity twice, so counting it as good hides a real loss. Track rework and scrap separately if you need to tell the two apart.

Is quality rate the same as first pass yield?

For a single operation, effectively yes. They diverge across a multi-step process, where first pass yield is usually rolled up by multiplying the yield of every step. That produces a much lower and more honest number than measuring the final step alone.
